Student-Athletes: It's Time to Get Your Sports Physical!
As we prepare for another exciting year of Walsh Eagles athletics, now is the time to make sure you are ready for the first day of practice. One of the most important requirements for participation is a current sports physical. Athletes may not participate in practice until a valid physical has been completed and the required paperwork has been submitted. We encourage families to schedule appointments as soon as possible to avoid last-minute delays and ensure students are eligible to begin practice on time.
According to CHSAA bylaw 1780.1, an adequate physical examination by a licensed practitioner is required within 365 days of sports participation.
Please download Preparticipation Physical Evaluation (PPE) pages 1, 2, 3 and 4.
Parent and athlete to both complete forms (Page 1, 2), including important medical information pertaining to your student athlete that the school will need to know.
Bring all four (4) pages to the licensed practitioner for physical examination in timely manner prior to sports season.
Student or parent shall submit
ONLY PAGE 4
to the school athletic office as part of the athletic registration process.
Athletes must also have the following forms signed and turned in to the high school office prior to the season. This packet of forms may be picked up from the high school office. They can also be found from the links below. .
